Our Assist Personal Activities High & Complex Supports services are designed to provide expert solutions for individuals with intensive support needs. Community Nursing Care For High Needs

We provide community nursing services to help individuals and families prevent disease, maintain health, and receive treatment. Our team offers overnight and general nursing care and complex wound management to ensure compassionate and practical support for each person’s unique needs.

Assist Access/ Maintain Employment

We provide valuable assistance to clients seeking employment. We help them acquire the necessary skills and knowledge to excel in their chosen career path. Our clients benefit from our professional network, which allows them to connect with potential employers and secure employment opportunities.

Assist -life Stages, Transition And Support.

Assistance with life-stage transition service includes short and long-term support that aims at strengthening and supporting the participant’sparticipant’s abilities and choices. We help you with these transitions, such as moving out of your home or getting a job are part of everyone’s life.

Assist -Travel/Transport

Travel and transportation assistance comprises assistance that enables participants to develop the capacity to travel independently, such as using a personal vehicle, related aids and equipment, or training to use public transportation alone.

Assist Daily Tasks/share Living

We assist the participant in fostering personal growth and expanding opportunities for community involvement. We facilitate them in utilising public transportation, acquiring skills, and performing daily tasks.

Innovative Community Participation

We work with you to develop goals, confidence and independence to feel empowered and get the most from life. We help participants focus on life skills and development activities and increase their ability to live independently.

Development Of Daily Living & Life Skills

We understand that personal hygiene, grooming, and other daily living activities can be challenging for individuals with complex care needs. That’s why we offer support in these areas to help promote their wellbeing and independence. Our team is always here to provide friendly and comforting assistance whenever needed.

Household Tasks

At Community Inclusion, we offer household help for disabled individuals, which includes basic cleaning in kitchens, bedrooms, bathrooms, laundry rooms, and toilets. We take care of all the needs and domestic activities that participants cannot do independently.

Participation In Community, Social And Civic Activities

We assist with participating in community activities, social outings, and events, which can be a great way to foster social connections and community engagement. We understand the importance of community interaction and how it can help in the development process.
